Creatine and Hair Loss: What That One 2009 Study Actually Found
Every few weeks someone sends us a version of the same message: "Is creatine going to make me lose my hair?" It usually comes from someone who already checks their hairline in every bathroom mirror, and it always traces back to the same source. One small study from 2009 that thousands of forum posts have been quoting ever since.
I went and read it. The distance between what that study measured and what people believe it proved is the entire reason this fear exists, so let's close that distance.
Does creatine cause hair loss?
There is currently no good evidence that creatine causes hair loss. Not one study has measured hair as an outcome and found that creatine users lost more of it. The entire concern rests on a single small trial that measured a hormone associated with pattern baldness, in a small group of young male athletes, over a few weeks, and never looked at anyone's scalp.
That is a real finding worth understanding. It is not the same thing as evidence that creatine thins hair, and treating it as such is how one blood marker turned into a decade of internet certainty. What did the 2009 creatine study actually find?
The 2009 study found that young male athletes taking creatine showed a rise in dihydrotestosterone, or DHT, a hormone linked to pattern hair loss, and it never measured hair at all.
The design, in plain terms: a small group of college-age male athletes, a short trial, a loading phase followed by a lower maintenance dose, and blood draws to check testosterone and DHT. DHT went up in the creatine group relative to placebo. The reported hormone levels were not described as moving outside the normal clinical range.
Here is what did not happen. Nobody counted hairs. Nobody measured hair thickness, shedding rate, or hairline position. No dermatologist evaluated a single scalp. Hair was never an endpoint of the study, which means the study cannot tell you anything about hair. It measured a signal that is one step upstream of a thing people care about, and the internet collapsed those steps into a headline.
Why does DHT get blamed for hair loss in the first place?
DHT is an androgen that, in people who inherit a genetic sensitivity to it, gradually shrinks hair follicles over years. That sensitivity is the operative word. Two men can walk around with similar circulating DHT and have completely different heads of hair at 45, because pattern baldness is driven far more by what your follicles are genetically primed to do with the hormone than by how much of it is floating around.
So a modest shift in a hormone marker, in a handful of young men, over a few weeks, tells you very little about what follicles do across decades. Same answer applies. The evidence is not there.
Has any other research replicated the DHT finding?
No. Follow-up research has not consistently reproduced the DHT change, and broader reviews of creatine's safety have not turned up a hair-loss signal. Creatine monohydrate is one of the most studied supplements in existence, with a long track record across many trials and populations. If it reliably drove hair loss, that would be an easy thing to catch by now.
A single unreplicated result is how science starts a question, not how it answers one.
What did the study measure versus what people think it measured?

What do we still not know?
We do not have a long-term trial that tracked hair density in people genetically prone to pattern baldness while they took creatine, so "no evidence of harm" is not the same sentence as "proven safe for your hairline." I would rather say that plainly than sell you certainty I don't have.
The honest unknowns:
- Whether a small, temporary hormone shift means anything at all over years of use. Probably not, but nobody has run that study.
- Whether people with strong genetic sensitivity respond differently than the general population.
- Whether a loading phase behaves differently from a steady daily dose over the long run.
Those gaps are the reason the reassuring answer stops at "no good evidence" rather than "impossible."
Should you take creatine if pattern baldness runs in your family?
If you have a family history of pattern hair loss and this genuinely worries you, talk to a dermatologist rather than a forum. A dermatologist can look at your actual follicles, establish a baseline, and tell you what your scalp is already doing independent of any supplement. That is information no blog post can give you, including this one.
A practical note if you decide to try creatine anyway: hair cycles are slow, so any real change takes months to show up, not weeks. Panic-quitting after two weeks tells you nothing.

How should you actually take creatine?
Five grams a day of creatine monohydrate is the research-standard dose, and muscle stores saturate in roughly three to four weeks at that dose, so loading is optional. The standard loading protocol, around 20g a day for five to seven days, gets you there in about a week instead, and the 2009 trial did include a loading phase of its own before dropping to a lower daily dose.
