Creatine With and Without the Workout: 64 Adults, 12 Weeks

Sixty-four adults between 45 and 65. Twelve weeks. Ten grams of creatine monohydrate a day, or a placebo. Half the group also took on an exercise and weight-loss diet program, and half changed nothing else at all.

That second half is the reason this trial is interesting. Most creatine research asks what creatine adds to training. This one also asked what it does for a sedentary middle-aged person who is not training, which is a question a lot of people actually have.

WHO WAS IN IT

Seventy-three healthy sedentary adults started. Sixty-four aged 45 to 65 completed the 12 week intervention and were analyzed, mean age 54.5 years, mean weight 84.5 kg. Forty were women and 24 were men, which is a better balance than most of the creatine literature manages.

Body composition was measured by DXA at weeks 0, 6 and 12, alongside fasting blood samples and a battery of cognitive tests and questionnaires.

WHAT THEY TOOK

2 x 5 g a day of creatine monohydrate, or a matching maltodextrin placebo. That is 10 grams a day, double the familiar 5 gram dose, and it is the first thing to hold onto before reading the results.

The structure had two layers. Participants first selected for themselves whether to join a non-exercise track or an exercise and diet track. Only then were they randomized, double blind, to creatine or placebo inside whichever track they had chosen.

WHAT THEY FOUND

The abstract reports directions rather than numbers, so that is what we can give you.

  • Creatine without exercise or diet changes: increased lean tissue mass, strength and muscular endurance, with favorable changes in selected blood lipids, HbA1c, and selected markers of cognitive function and memory
  • Creatine during the exercise and weight-loss diet: increased lean tissue mass, produced a greater reduction in body fat percentage, and improved muscular strength, endurance, and selected markers of cognitive function and memory
  • Tolerance: supplementation was well tolerated

The authors' conclusion keeps its hedge in the open. Creatine "may help middle-aged and older adults maintain muscle mass and strength while favourably affecting selected markers of health and cognitive function." May help. Maintain.

WHAT IT DOES NOT SHOW

  • People picked their own track. The randomization was to creatine or placebo, not to exercise or no exercise. Participants chose that part themselves, so the two tracks are not a randomized comparison and you cannot read one against the other as though they were
  • No effect sizes are reported in the abstract. Every result above is a direction with no number, no confidence interval, and no p-value attached to it. We are not going to supply numbers the record does not print, so we cannot tell you how large any of this was
  • The dose was 10 grams a day, not 5. Twice the amount most people take. Nothing here transfers to a 5 gram routine by assumption
  • The word "selected" is doing real work. Selected blood lipids, selected markers of cognitive function. Some measures moved. The abstract does not say which ones, or how many did not
  • The authors flag their own cognitive findings. In their words, "Individual cognitive and biomarker findings should be interpreted as exploratory." That is the researchers telling you not to lean on the brain results, and we are passing it along rather than burying it
  • Nine of the 73 who started are not in the analysis. The abstract does not say why they left
  • Twelve weeks. Long enough to move body composition, not long enough to say anything about a year or a decade

Funding and conflicts, stated plainly: senior author R.B. Kreider discloses industry-sponsored creatine research, chairs the scientific advisory board for AlzChem, a company that manufactures creatine monohydrate, co-founded the nonprofit society that publishes this journal, and sits on two other nutrition advisory boards. The other twelve authors report no conflicts. All of that is disclosed in the paper. It does not void the trial and you should not have to dig for it.

WHY IT IS ON THIS SHELF

Because of who was in it. Fifty-four years old on average, sedentary at the start, and mostly women. That is much closer to the person reading this than the young male athletes who fill most of the creatine literature.

And because of the question the non-exercise track asks out loud: what happens if you take creatine and do not otherwise change your life? This trial reports movement in that group. It also used ten grams a day to get there, and it could not randomize who trained, so treat it as an encouraging read rather than a settled one.
